Patient Health Forms provide a flexible and user-friendly way to manage patient information, offering pre-built templates and robust customization options. These forms are designed to work seamlessly with SOAP notes and other practitioner tools, ensuring that patient data is collected efficiently and accurately.

Key Features

  • Pre-Built Templates: Start with default forms like "Initial Health History" or "Follow-Up Forms," which include automations and pre-configured indexes such as Problem Index, Medications, and Women's Health.

  • Customization Options: Modify forms, reorder sections, add or remove indexes, and even build new forms from scratch to suit your practice’s needs.

  • Automation Integration: Set up forms to be sent automatically with specific appointments or trigger forms at specific intervals, like every third visit.

  • Patient Accessibility: Patients can fill out forms via computer, tablet, or phone with a beautifully formatted interface for higher compliance.

  • Additional Questions and Portals: Add custom questions like referral sources or insurance details directly in the patient portal, enhancing data collection.


How It Works

  • Access Forms: Navigate to the Settings > Notes > Patient Health Forms section.

  • Edit and Customize: Use the actions button to preview, edit, duplicate, or deactivate forms. Add new indexes or rearrange sections as needed.

  • Automation Setup: Attach forms to specific appointments or create rules for when forms are sent (e.g., first visit, follow-ups).

  • Send Forms to Patients: Assign forms directly via the schedule or patient demographics page. Forms can also be sent automatically based on appointment type.

  • Patient Interaction: Patients can complete forms on their devices, including updating previous data, adding notes, or attaching images.
